Application Steps

• Become a CLSA Member
• Pass the National Contact Lens Exam
• Complete one (1) of the following Professional Development Aspects:
  • Two (2) case histories
  • Publish an article in a trade journal
  • Give an accredited lecture at a meeting
• Pass The CLSA Fellow Practical Exam (3 parts)
  • Inspection and verification
  • Slide identification
  • Clinical analysis

Send a "Letter of Intent" to the CLSA office, marked "Attention: Fellow Chair. to begin the process. A mentor will be assigned to help guide you through to success!
